Clinical significance of Colonic Transit Time Study for Diagnosis in Patients with Chronic Constipation

Abstract
Purpose : A test for measuring total and segmental colonic transit time is a useful tool in the evaluation of patients with complaints of chronic constipation. However,, lack of published data regarding the information of colonic transit time study(CTTS) has limited a more clinical usage. Therefore, the aim of this study was undertaken to assess whe~ier CTTS is helpful for diagnosis in patients with" chronic constipation.

Methods : Between October 1998 and September 1999, all patients who underwent CTTS and cinedefecography were evaluated. CTTS was performed by method of single marker bolus technique; taking twenty-four markers on the day 1, then two abdominal films on the day 3 and 5: Types of CTTS were classified according to distributions of radiopaque markers; slow transit, outlet obstruction, indeterminate, and normal transit. Findings of CTTS are compared with cinedefecographic findings.

Results : 23 patients with chronic constipation, 8 males and 15 females, with a mean age of 46.516.7 years were underwent CTTS and cinedefecography. The mean duration of symptoms was 7.83.7 years. As for the 5th day findings of CTTS, 5 cases(21.7%) were abnormal. Among these patients, 2 cases(8.7%) were slow transit, 2 cases(8.7%) were outlet obstruction, and 1 case(4.3%) was indeterminate type. As for the 3rd day findings, 11 cases(47.8%) with slow transit(2 cases), outlet obstruction(3 cases), indeterminate(6 cases) were abnormal. Cinedefecographic findings showed 10 cases(43.3%) of outlet obstruction pattern, and there were significant ~g<005).

Conclusions : The sensitivity of compared to 3rd day and 5th day¢¥s CTTS findangs(P However, this test could be performed CTTS was suboptimal as compared to cinedefecoaraphy. patterns of as initial test for diagnosis of chronic constipation because it can classify the constipation.
